The Patient Portal is a secure web-based system that allows patients to contact our office electronically bypassing voicemails whenever and wherever it is most convenient for you!

Just call 815-398-9491 or stop in person at our front desk and let us know you want to use the new Patient Portal services. In less than a minute we'll issue you a user name and password that will connect you to the convenience of:

  • Post non-urgent* questions or requests for your physician
  • Receive communications back from us
  • Request an appointment
  • Access your account information to update changes to your contact, preferred pharmacy, employer, and insurance provider information
  • Complete medical history information and other questionnaires online prior to your appointment.
